Registration or intimation of claim in HDFC Life
For Intimation of claim online you need below details,
- Policy Number
- Life Assured Date of Birth
- Life Assured Date of Death
- Mobile Number
- PAN Number
- Claimant’s Mobile Number
Documents required for Death Claim at HDFC Life
1) For Natural Death :
If death has happened naturally then you need to produce the following documents for claim.
- Death certificate issued by Govt. / Relevant Authority.
- Death claim form (includes NEFT).
- Original Policy Document (In Case of DEMAT, Original Policy Documents are not Required).
- Claimant’s Residence Proof.
- Claimant’s Photo ID proof and/or PAN card.
- Claimant’s photograph.
- Medical records at the time of death & past illnesses.
- Personalized Canceled Cheque or Bank Passbook.
- Employer’s certificate (Form), if employed.
2) For Un-natural Death (Accidental death / Murder / Suicide) :
If death has happened un-naturally then you need to produce the following documents for claim.
- Death certificate issued by Govt. / Relevant Authority.
- Death claim form (includes NEFT).
- Original Policy Document (In Case of DEMAT, Original Policy Documents are not Required).
- Claimant’s Residence Proof.
- Claimant’s Photo ID proof and/or PAN card.
- Claimant’s Photograph.
- FIR, Police Inquest Report & Panchanama.
- Post Mortem Report.
- Personalized Canceled Cheque or Bank Passbook.
- Employer’s certificate (Form), if employed.
3) For Disaster / Natural Calamities
If death has happened due to any disaster or natural calamities then you need to produce the following documents for claim.
- Death certificate issued by Govt. / Relevant Authority
- Death claim form (includes NEFT)
- Original Policy Document (In Case of DEMAT, Original Policy Documents are not Required)
- Claimant’s Residence Proof
- Claimant’s Photo ID proof and/or PAN card
