- Step 1 : Claim Intimation

- Click on file a claim online.
- Enter policy number, insured person’s name, nominee relationship, DOB of the insured person and hit the Add Policy button to proceed.
- Enter Captcha and hit the submit button.
- Step 2 : Document Submission
- Once the claim has been intimated, you will be asked to submit the mandatory documents to process your claim further.
- Step : Claim Settlement and Payout
- Submitted documents will be assessed, investigated and a claim decision will be taken.
- Payout will be made through NEFT as per T&C of the policy.

Documents required for Death Claim at PNB MetLife Insurance
For Natural Death :
Nominees have to submit the following documents along with a completely filled claim intimation form.
- Claim Form
- Death certificate issued by local authority
- Treating Doctor’s Certificate
- Current address proof of the nominee
- Photo ID proof of the nominee
- Cancelled cheque / Copy of bank passbook
- Authorization letter from the claimant in case the claim intimation is received through third party for claims received at the branch/GPH
- Complete Medical records (Admission notes & Discharge / Death summary & Test/ Investigation reports etc.)
- Legal heir/Succession certificate in case of absence of nominee
For Accidental Death :
In addition to the above documents, you need to submit the following documents for accidental death.
- Copy of FIR, Panchnama, Inquest report, Postmortem report
- Obituary/Newspaper cutting (if available)
- Viscera / Chemical analysis report (if applicable)
- Final police investigation report
- Legal heir/Succession certificate in case of absence of nominee

Please Note that Critical Illness can be claimed only once the Survival Period is completed. You need the following documents for critical illness claims.
