For Medical / Natural Death :
Nominees have to submit the following documents for medical or natural death.
- Duly filled and signed Claim Form
- Death Certificate of the policyholder
- Photo ID, Address proof and relationship with policyholder proof
- Original copy of a Canceled Cheque having the name of the nominee
- Original Policy Papers
- Photocopy of Passbook or Bank Statement which is not more than 3 months old. (attested by the branch manager or gazette officer)
